I need personalized disposable chopsticks for my business.?

I need custom disposable chopsticks for my restaurant. I'd like the sleeve to be completed closed with the premium set of disposable chopsticks and have my business name and logo with address and phone number on one side and out website on the other. I found one website that would do a box of 200 for 55 dollars but I lost the bookmark. Can anyone help me?
